How to make a self referral to us
We have no formal referral process to allow ease of access for parents. All you need to do is contact us via any of the methods on our “Contact Us” page (preferably email), and provide all of the following details. This will help speed up the process of answering queries:
- Parent/Carer full name
- Parent/Carer email address
- Parent/Carer phone number
- Parent/Carer home address and postcode
- Child's full name
- Child's gender
- Child's date of birth
- Child's ethnicity
- Child's home address and postcode if different
- Child's education setting (e.g. name of nursery, school, college)
- Child's needs (e.g. diagnoses, SEN, suspected conditions, disabilities)
- Reason for referral/outline of enquiry
